Meet Carol

Carol is 62 and lives in a small community just outside Kingston. She has high blood pressure and struggles with anxiety, and she was recently diagnosed with type 2 diabetes. For years, she saw a doctor in another town but often had trouble getting appointments. When she needed help managing her conditions or accessing mental health support, she didn’t know where to start — and often gave up trying.

Since being connected to a Health Home in her neighbourhood, things have changed. She sees her nurse practitioner for regular check-ups, and now she’s also connected to a mental health counsellor and a care coordinator who helps her manage her diabetes, medications, and appointments. Her care team shares information and works together, so she doesn’t have to repeat her story or navigate the system alone.

For Carol, the Health Home means she finally feels supported. She gets care that’s connected, personalized, and close to home — and for the first time in a long time, she feels confident managing her health.

Meet Jordan

Jordan is 28 and recently moved to Napanee for work. They’ve been living with asthma since childhood and have been feeling increasingly anxious, but haven’t had a regular doctor since moving. Without a provider, Jordan often relied on walk-in clinics for prescriptions and avoided seeking mental health support because it felt too hard to figure out where to start.

Through Health Care Connect, Jordan was connected to a new Health Home in their community. Now, they have a nurse practitioner who helps manage their asthma and a care coordinator who introduced them to a local counsellor for anxiety. The team works together and checks in regularly, which has helped Jordan stay on top of their health and feel less alone.

For Jordan, the Health Home has taken the stress out of finding care. With a team that understands their needs and works together, Jordan finally feels like they have a place to turn for both physical and mental health support.

Meet Amira

Amira recently moved to Kingston with her two children. As a newcomer to Canada, she wasn’t familiar with how the health-care system works and didn’t have a primary care provider. When her youngest child developed a rash and she started having trouble sleeping from stress, she wasn’t sure where to go or who to talk to. She often delayed seeking help because the process felt overwhelming.

Amira was connected to a local Health Home. Now, she has a primary care team that helps her manage her own health concerns and provides care for her children as well. A care coordinator supports her with appointment scheduling, translation services, and access to community programs for newcomers. Her care team takes the time to explain things clearly, listen to her concerns, and respect her cultural preferences.

For Amira, the Health Home has made a world of difference. She feels welcomed, supported, and no longer has to navigate the system on her own. With a team that cares and communicates, she’s able to focus on her health and her family’s wellbeing.
