CDK Family Medicine Health Home is a family medicine practice at three locations. The walk-in clinic was designed to increase accessibility of healthcare to those without family doctors and to lessen the burden of non-emergent issues flooding the Urgent Care clinic. The after hours clinic is only open to patients already registered (rostered) to the Health Home's family doctors.

  • CDK Family Medicine and Walk-in Clinics:
    • CDK Downtown: (Walk-in clinic and rostered patients)
      • 175 Princess St., Kingston, ON K7L 1A9 
      • Monday - Friday: 9 am - 5 pm, Saturday - Sunday: 10 am - 2 pm
    • CDK Sutherland: (Rostered patients only, no walk-ins) 
      • 105 Sutherland Drive, Kingston, ON, K7K 5V6
      • Monday - Friday: 9 am - 5 pm
  • Princess Street Medical Centre: (primary care and allied health for rostered patients only)

2. Then wait for our office to contact you to complete our intake forms and set up a first appointment.
