CDK Kingston Health Home
Accepting new people for care close to home!
The FLA OHT and its partners are continuing to help attach people who do not have a primary care provider (family doctor or nurse practitioner) to a care team close to home.
Starting in mid-April the CDK Kingston Health Home will roster another 2650 people who do not have a primary care provider (including family members living at the same address) living in their geographical areas. Over 1500 people have already been rostered since February 2024.
Residents must have government issued ID as proof that they live within the Health Home catchment boundaries (see map below):
- North: Highway 401
- South: Lake Ontario
- East: Cataraqui River
- West: Sir John A. Macdonald Blvd (east side)
Steps for rostering to the expanding Health Home:
- Register with Health Care Connect (More Health Care Connect information)
- Visit the CDK website for information about rostering days and the required forms to complete.
Please do not call the clinic to inquire about their rostering status. People are asked to be patient as staff work hard to roster registrants to their clinic. Residents eligible to become attached will be contacted by their new Health Home and/or by Health Care Connect with further information on accessing care.
