Primary Care Network

The Primary Care Network (PCN) is a key part of the Frontenac, Lennox & Addington Ontario Health Team (FLA OHT). It is a group of local primary care providers (family doctors and nurse practitioners), hospital leadership, and community partners working together to improve primary care across the Frontenac, Lennox, and Addington region. 

We believe people get better care when providers are connected, share information and plan together. That’s what the PCN is all about – bringing care closer to home, making it easier to get, and making sure no one is left behind. 

Current challenges

Across the FLA region, many people are still unattached, meaning they don’t have a dedicated primary care provider. Primary care is often the first place many people go when they need help with their health. But today, there are big challenges: 

  • Many people don’t have a family doctor or nurse practitioner 
  • Primary care providers are stretched thin and facing burnout
  • There’s a gap between what people need and what the system can offer

These challenges can lead to longer wait times, difficulty accessing care when they need it, and more stress – for both people and providers. 

What we are working towards

The Primary Care Network is finding solutions, together. Our goals include: 

  • Connecting people to a Health Home – a local primary care team that knows them and can support them. 
  • Making care more equitable, no matter where you live or what your needs are. 
  • Improving how the system works together from primary care clinics to hospitals to home care and social supports.
  • Using local data to plan better services in neighbourhoods across our region. 
  • Building a strong unified voice for primary care within the broader health system.
  • Helping to implement digital tools and develop new supports for primary care teams. 

Our Primary Care Network Projects

Developed with input from our primary care providers and community partners:

  • Developing the Health Home Framework 
  • Attaching people without a primary care doctor to care 
  • Supporting family doctor and nurse practitioner recruitment and retention 
  • Helping plan care so services and resources reflect the local needs
  • Improving home and community care in partnership with Ontario Health at Home
  • Supporting primary care teams with shared tools and digital resources
