Message from the Executive Lead
When we embarked on our five-year strategic plan, Achieving our best health, the first leg of our journey was focused on bringing partners together—laying a foundation built on trust, shared purpose, and open dialogue across people, communities, primary care, hospitals, and service organizations of many types. Today, as we stand at the midway mark of our five-year strategy, our focus has intentionally shifted: building on the foundation of partnerships to demonstrating collective impact.
Over the past year, our shared vision has translated into tangible progress across our region. Through true collaboration, we achieved major attachment milestones, expanding team-based primary care access and connecting thousands of unattached community members to People-Centred Health Homes. But primary care attachment is just one piece of the puzzle. Every initiative highlighted in this report reflects how our network is working as one connected system to deliver the right care, in the right place, at the right time.
Our momentum these past few years was directly shaped by the collective wisdom of our partners and community. At our TLC Retreat and All-Partner Town Hall in Spring 2026, providers, leadership, and community members gathered to evaluate our progress and co-design the next stretch of our journey. Together, we refined our priorities to ensure our work directly responds to local health needs, removes barriers to care, and supports our health human resources.

Message from the Community Council Co-Chairs
As Co-Chairs of the FLA OHT Community Council—and as patients, family members, caregivers, and neighbors ourselves—we are so proud to share what together with all members of Community Council, we have accomplished together this past year.
At the end of the day, health care isn't just about services and systems. It’s also about people and perspectives. That is why the FLA OHT’s strategy centers on a simple truth: the best health care is co-designed with the people who actually use it. We aren’t just sitting on the sidelines receiving care—we are at the table working as equal partners, informing design by sharing our lived experiences to help shape how care works across Frontenac, Lennox, and Addington.
This past year showed what is possible when our local communities lead the way. From testing and shaping new tools like the Find Services search tool, to guiding equity efforts, Indigenous cultural safety, and end-of-life care, community voices were woven into every major project.
The stories featured throughout this digital report highlight how we brought this vision to life. As you read through the spotlights and achievements, you will meet the dedicated individuals, partners, and advocates who are helping us build a more compassionate, accessible, and connected health-care system for everyone. Before care comes listening: FLA OHT and Indigenous palliative care
April 15, 2026. Last Fall a collaborative, community-engaged effort brought together many FLA OHTpartners for a Talking Circle focused on Indigenous palliative and end-of-life care. The goal was simple: to listen.

December 11, 2025. Community members of the FLA OHT are not just advisors, they are co-designers whose insights, lived experience and perspectives help shape the new model of health-care delivery in the region.

June 10, 2025. Across our region,health-care providers are coming together to strengthen gender-affirming care and support health equity through a growing regional community of practice known as the Gender Diversity Rounds.

FLA OHT receives $3.62 million to attach 8,500 people to primary care
May 20, 2026. Funding was awarded to the FLA OHT, with partner the Maple Family Health Team, focused on attaching up to 8,500 community members to a family physician or nurse practitioner by the summer of 2027.
Palliative care leaders gather to shape future end-of-life care
April 15, 2026. A workshop took place in March with a goal to build a more connected, responsive and compassionate palliative care system for residents of the Frontenac, Lennox and Addington (FLA) region.
For providers navigating end-of-life care, a guide to help
April 14, 2026. When managing people's end-of-life care, providers-physicians, nurse practitioners, nurses and care coordinators, and allied health providers alike-have a resource to support their journey.
FLA OHT introduces online hub for palliative care resources
February 17, 2026. Two new online hubs support patients, caregivers, families and health-care providers by centralizing palliative care resources, such as practical tools and links to local services.
Holiday Health Resource List helps finding care over the winter break
December 15, 2025. The FLA OHT has released its annual list of Holiday Health Resources to help community members know where and how to access care between December 22, 2025 and January 5, 2026.
FLA OHT receives $1.49 million to attach 3,500 people to primary care
October 21, 2025. The FLA OHT received $1,495,100 from the Ontario government to attach up to 3,500 community members in the region to a publicly funded family doctor or primary care team by April 2026.
Integrated Care Pathways help people with COPD and heart failure stay healthier, longer
April 16, 2025. FLA OHT partners are finding innovative ways for providing care to people living with certain chronic diseases who do not have a primary care provider.

April 15, 2025. The FLA OHT and its partners are breaking new ground with an innovative initiative designed to boost collaboration and empower paramedics to support heart failure care.
