Beyond these guidelines, the FLA OHT Priority Work Group on Integrated Care Pathways for Chronic Disease identified two conditions - Heart failure and Chronic Obstructive Pulmonary Disease - as priorities in the region. These FLA OHT partners comprised of clinical experts and community service providers have been working at developing integrated, wrap-around care pathways the people we serve.
