Digital Innovation & Practice Enablement
- Funding and technical support (via the FLA OHT) to help practices adopt digital tools that improve efficiency and person-centre care.
- Support includes AI medical scribes, eReferral and eConsult, online appointment booking, EMR custom forms and tools (via OntarioMD)
- Regular updates & opportunities shared through the Primary Care Physicians Council.
- Access to the FLA OHT Digital Support Structure, offering hands-on guidance to adopt new tools and optimize workflows.
Connected Community of Colleagues
- A network of peers who meet regularly to discuss shared challenges, best practices, and solutions for effective, efficient practice management, system navigation, access to subspecialists and referral pathways, public health resources and updates
- A forum to surface issues, identify trends, and collaboratively influence system improvements.

- Unified applications and advocacy for funding to support team-based care expansion, practice facilitation, digital innovation quality improvement

- Strengthened communication pathways between primary care teams (family doctors and nurse practitioners, hospitals, community mental health and addictions services, public health, home and community care, subspecialists and community support agencies

Extension of Community Healthcare Outcomes - ECHO - is an interactive online medical education program, connecting health care providers with an interprofessional specialist team to advance learning and enhance clinical skills.
Stroke Network of Southeastern Ontario
The Stroke Network of Southeastern Ontario is one of 11 regional networks established across the province to advance stroke prevention and care. They offer many professional resources to improve access to evidence-based education, guidance and tools.
Prenatal Screening Ontario
Prenatal Screening Ontario (PSO), funded by the Government of Ontario, coordinates the operation of prenatal screening services in the province. Along with information for individuals, PSO offers screening requisitions, education opportunities and other resources for practitioners.
AccessMHA: mental health, substance use health & addictions services
AccessMHA provides a single point of referral for mental health, substance use health, and addictions services. It is available to individuals aged 16 or older at no cost.
