What is a Palliative Care Clinical Coach?

A Palliative Care Clinical Coach is a healthcare professional who provides expert guidance, mentorship, and support to healthcare teams across diverse care settings. Palliative Care Clinical Coaches collaborate closely with the Ontario Health Team and the Ontario Palliative Care Network in their region.

Eligible organizations

Palliative Care Clinical Coaches collaborate with various community health organizations, including long-term care homes, retirement homes, home care providers, primary care practices, and community paramedicine programs. Together, we can enhance the delivery of palliative care services and support best practices in patient-centered care.

How can we help?

Palliative Care Clinical Coaches assist organizations by assessing strengths and gaps, facilitating access to funded education, supporting quality improvement initiatives, mentoring teams, offering feedback on complex cases, and fostering connections within the broader healthcare community.

OPCN Model of Care implementation

The OPCN Model of Care is a framework designed for well-organized palliative care, encompassing 13 key recommendations to guide the organization and delivery of care for adults in the community. Palliative Care Clinical Coaches play a vital role in supporting the adoption of this model, helping organizations move toward providing the ideal standard of care.

Supporting competency

Palliative Care Clinical Coaches focus on improving access to and the quality of clinical care while strengthening connections with specialist providers. Our work is aligned with the Palliative Care Health Services Delivery Framework and the Ontario Palliative Care Competency Framework.
