FLA OHT and Partners Connect 5,500 Individuals to Health Homes, Expanding Access to Comprehensive Team-Based Care

The Frontenac, Lennox and Addington Ontario Health Team (FLA OHT) is pleased to share significant progress in expanding access to primary care and advancing the Health Home model across the region with the release of its inaugural annual report for April 1, 2023 to March 31, 2024.

This report highlights the collaborative achievements of the FLA OHT and its partners, who have successfully attached approximately 5,500 individuals to Health Homes, where they receive comprehensive, team-based care tailored to their unique needs. This includes the attachment of more than 650 Indigenous Peoples through the Indigenous Interprofessional Primary Care Team and more than 700 pregnant women and their newborns through the Maternal and Child Health Clinics. Additionally, the recent attachment of 3,400 people to the CDK Kingston Health Home, and 500 and 200 people to the East End and Greater Napanee Health Homes, respectively, further underscores these efforts.

This year's report highlights additional successes reflecting the FLA OHT's commitment to enhancing health-care across the region, including:

  • Enhanced primary care services by providing over 300 care encounters through the Well Baby Care Clinic, and about 950 sexual health-care encounters
  • Improved virtual care and implemented digital health tools to enhance patient-provider communication by providing over 30,000 people with access to online appointment bookings
  • Reduced administrative burdens and advanced electronic medical record systems to streamline care by replacing faxes with digital reports allowing providers to focus more on patient care, fostering a more supportive and effective health-care environment.

These initiatives, alongside many others, underscore a year of significant progress in improving health-care accessibility, quality and outcomes for FLA community members.
