We are excited to share some of the achievements resulting from our FLA OHT partner collaborations, which are improving the lives of people in our community. In 2024, we continue our momentum on existing initiatives and include three key integrated care initiatives in the coming year: maternal and child health care, integrated care pathways for chronic disease and preventative care and disease screening.

October 2023. This month we recognize and champion the work our partners are doing to support people in their cancer journeys. Encouraging participation in cancer screening is vital to keep people healthy and reduce the burden of the disease, but when people are diagnosed with cancer, collaborative, integrated care has the power to help people recover, or support them well at the end of their lives.

The FLA OHT has many projects and initiatives moving full steam ahead. By offering education and support strategies for people to take charge of their own health,  our primary-care teams help identify and intervene early in disease processes and create seamless connections to specialized care, such as cardiovascular care,  to influence the course of chronic disease progression.
