Achieving our best health, together: Demonstrating our collective impact
The Frontenac, Lennox and Addington Ontario Health Team (FLA OHT) is excited to share the release of its Annual Report for April 1, 2025, to March 31, 2026
At the midpoint of the five-year strategic plan, the FLA OHT report highlights how building a strong foundation with our partners – including community members, communities, primary care teams, hospitals, and service organizations of many types – has enabled us to collectively re-design our health-care system.
“Over the past year, our shared vision has translated into tangible progress across our region. Through true collaboration, we achieved major attachment milestones, expanding team-based primary care access and connecting thousands of unattached community members to People-Centred Health Homes. But primary care attachment is just one piece of the puzzle. Every initiative highlighted in this report reflects how our network is working as one connected system to deliver the right care, in the right place, at the right time”, says Dr. Kim Morrison, Executive Lead, FLA OHT.
The FLA OHT’s impact is demonstrated through achievements such as attaching 6,500+ people to a family doctor or nurse practitioner and a 49% reduction in ER visits for people with heart failure. These results reflect the immense dedication of our regional partners - primary care providers, physicians and nurse practitioners, and team members, who work every day to connect unattached people to primary care teams to support people to achieve their best health.
This Annual Report also celebrates the vital contribution of Community Council members and spotlights members who joined during the 2025-2026 period. Brenda Luffman and Kerry Stewart, Community Council Co-Chairs, reflect on this collaborative work with great pride.
