Maple Family Health Team

Positions: Two well-managed full-time practices of over 2,000 patients each, each of these rosters could be split into smaller practices.

  • Ideally starting July 2024, however a later date up to July 2025 can be negotiated

Maple Family Health Team  (https://maplefht.ca/) - We have several sites; this posting is for:

  • Rideau Family Medical Centre
    • 235 Gore Road, Kingston, Ontario K7L 0C3

Rideau Family Medical Centre is a small clinic with two large practices in Kingston East.  It is located in a well-established shopping plaza adjacent to a large neighbourhood. 

The collaborative team includes a nurse practitioner, two registered nurses, two medical secretaries and one office manager.  There is ample access to Maple FHT’s After Hours Clinic and other providers, including mental health counsellors, dietitians, diabetes educators, a respiratory educator, and a community service worker.
