FLA OHT Maternal Child Health Strategy
The maternal child health strategy is informed by years of experience in primary care, particularly through listening to patients struggling with complex mental health and addictions. It aims to help parents get connected to community supports and resources so they can enter parenthood with a cocoon of care available to them and their children.
Increasing access to primary care close to home
The Frontenac, Lennox and Addington Ontario Health Team (FLA OHT) and its partners are working to provide care close to home. Up to 9,000 people who do not have a primary care provider (family doctor or nurse practitioner) will be attached to care teams close to home within the next six to 12 months
Expanding health care access in 2024
As we look ahead to 2024, the FLA OHT will work on three key integrated care initiatives: maternal and child health care, integrated care pathways for chronic disease and preventative care and disease screening.
Our first 18 months of success!
It’s been an exhilarating ride as the FLA OHT has evolved over our first 18 months. Our rocket ship has a solid structure and is hurdling towards our North Star of the People-Centered Health Home.
Celebrating the power of communities
It is well understood that people, together with their families, friends, neighbours and caregivers, provide the vast majority of care that keeps them well and healthy in their homes.
Health Home for infants without a primary care provider
The Well Baby Care clinic is addressing a large gap in our community for essential care of infants and young children that do not have a family doctor or nurse practitioner.
