Queen's Family Health Team - Pharmacist
Position: QFHT Pharmacist
- Full-time permanent position
- 35 hours/week
- Salary $56.81 - $60.27/Hour

- The QFHT Pharmacist will: practice patient-centred care to achieve optimal drug therapy outcomes for enrolled patients, as part of a team of primary care health professionals; provide pharmaceutical care to patients by optimizing medication therapy and acting as a resource to the team; and provide regular educational updates with respect to drug-related guidelines and evidence-based research to patients, physicians, residents and other professional staff within the clinic.

The Queen’s Family Health Team is a multidisciplinary team of health professionals who provide primary health care services to the Kingston community, who train future primary health care professionals and who conduct research in issues of primary health care delivery. The Queen’s Family Health Team is comprised of a number of medical practices and provides for a diverse, client centred work environment for all employees.
