Increasing access to team-based primary care

A new Health Home is coming! 

The province has invested $4 million into a new Health Home in the City of Kingston as part of the Frontenac, Lennox and Addington Ontario Health Team (FLA OHT).

This new Health Home will be a home base for easy access to all the health care and wellness services we need to achieve our best health.  It will be our front door to the health-care system, including a team of health-care professionals dedicated to supporting our best health and wellness through every stage of our lives. 

People will be enrolled in the new Health Home using Health Care Connect, their place of residence and specific health needs. 

If you don’t have a primary care provider (family doctor or nurse practitioner) you can register with Health Care Connect online or by calling 1-800-445-1822. The Health Care Connect registry will help connect everyone to a Health Home.
