Enhancing Community Care with a Team-Based Approach

Under the co-leadership of Dr. Ziny Yen, CDK is advancing community health, in part through their transformation into a People-Centred Health Home. This transformation involves expanding services, recruiting new staff, and successfully attaching people to primary care providers, all without additional funding. This success highlights the power of collaboration and strategic partnerships.

CDK clinic operates three distinct sites - the Princess Street walk-in clinic, the Sutherland clinic, and CDK Kingston Health Home - each playing a vital role in connecting the community to essential primary care services. Since May 2024, these clinics have collectively connected 3,420 individuals, approaching their target of 4,150.

  • The Princess Street Walk-In Clinic offers immediate care for people without a primary care provider and handles urgent, non-emergency issues; alleviating pressure on emergency services.
  • The Sutherland Clinic provides comprehensive primary care across all life stages ensuring continuity of care while being accessible for same-day urgent needs.
  • The CDK Kingston Health Home provides team-based care, including a physiotherapist, psychotherapist, mental health worker, Certified Diabetes Educators, and are recruiting a social worker and dietitian, ensuring comprehensive and holistic support.

In collaboration with the FLA OHT, CDK physicians are accepting new patients and integrating Health Homes within geographic areas to ensure unattached people have access to primary care close to home. Register for Heath Care Connect if you haven’t already in order to be connected to a primary care provider in a team-based Health Home in your neighbourhood as provider capacity allows.
