Expanding access to primary care in Frontenac, Lennox & Addington
Good news! The Frontenac, Lennox & Addington Ontario Health Team (FLA OHT) is working to connect more people to a family doctor or nurse practitioner through team-based People-Centred Health Homes.
The FLA OHT received $1,495,100 from the Ontario government to attach up to 3,500 community members in the region to a primary care team by April 2026. This funding is part of Ontario’s $2.1-billion Primary Care Action Plan, which aims to connect 300,000 people to primary care across the province this year. Funding was awarded through a provincial call for proposals.

What’s happening:
According to the most recent Ontario Health data, as September 2025 there are 10,751 people on the Health Care Connect waiting list and over 29,000 people who don't have a primary care provider in the FLA region. Here is how the FLA OHT and partners are working on it:
- What: A regional plan to attach up to 3,500 people to a family doctor or nurse practitioner by April 2026.
- Who: Frontenac, Lennox & Addington Ontario Health Team (FLA OHT) with Maple Family Health Team as lead partner.
- Where: Across the FLA OHT region.
- When: Now through April 2026–attachments occur as space becomes available.
- Why: To ensure everyone has access to team-based, people-centred care close to home.
- Funding: $1,495,100 this year through Ontario’s Primary Care Action Plan.
- Attachments to date: More than 13,000 residents were successfully connected to primary care providers (family doctor or nurse practitioner) across the region in 2024. That's progress!
What does “attachment” mean?
Being attached means having an ongoing relationship with a family doctor or nurse practitioner (and their team). They know your health history, provide most of your care, and connect you with other supports as needed.
I don’t have a family doctor or nurse practitioner. What should I do?
- Register with Health Care Connect or call 1-800-445-1822.
- Once registered, you’ll be matched to a Health Home (a primary care team) close to where you live as space allows.
- There is no set timeline for individual attachments, but priority is given based on:

- Proximity (care close to home)
- Health and equity needs
- Registration with Health Care Connect
The best next step is to make sure you’re on the Health Care Connect list — this is how people will be matched to providers.
Who will be attached first?
Priority groups include:
- People registered with Health Care Connect
- Pregnant people, newborns, infants, cancer patients, and others with urgent or complex needs
I’m on the Health Care Connect waitlist; is there a timeline for when I’ll get attached to a primary care provider?
There is no set date for individual attachments. 3,500 people will be attached by April 2026, and further attachments will happen as clinics have space—with a goal of attaching everyone in the region by 2029.
How are attachments decided?
Matches are based on where you live, your health needs, and available space in existing Health Homes and primary care teams.
What if I have a doctor outside of the FLA region, how can I change providers?
If you don’t have a primary care provider (doctor) close to home, register with Health Care Connect to be connected to one nearby.
What’s the “interprofessional health hub”?
The hub is a shared interprofessional health team that helps Health Homes serve more people. It offers access to:
- Mental health supports
- Community Support Worker / Navigator
- Physiotherapy
- Chronic disease management (e.g., diabetes)
What’s next for the region?
Future work will:

- Add new teams and clinic space where access is currently limited
