FLA OHT and partners connect thousands to primary care, clearing pre-January 1, 2025, Health Care Connect list

The Frontenac, Lennox and Addington Ontario Health Team (FLA OHT) and its regional partners have achieved a major health-care milestone, successfully clearing the entire pre–January 1, 2025, Health Care Connect (HCC) waitlist, connecting thousands of residents to primary care close to home.
According to Provincial data, there were 11,240 people on the regional waitlist as of January 1, 2025. Through coordinated efforts with primary care providers, Health Care Connect and the FLA OHT, 6,320 of those individuals were attached to a family doctor or nurse practitioner close to home. About 4,900 individuals were removed from the list after Health Care Connect confirmed they no longer needed a provider, ensuring local waitlists are accurate and up to date.
"Clearing this list is a significant milestone for our region and a beautiful reflection of what happens when partners create solutions together," said Dr. Kim Morrison, Executive Lead of the FLA OHT. "By working as a connected network, we’ve been able to achieve our vision of care close to home for thousands of our neighbours, ensuring they get the right care, right in their own communities. But our work doesn't stop here. We remain fully committed to attaching all residents in the region to accessible primary care."
A major part of this success was the opening of the new Spruce Health Hub in Kingston. This hub provides services to support family doctors across the region, helping them care for more patients. While someone waits to be connected to a family doctor or nurse practitioner, the Hub also provides temporary care and support. So far, about 400 people have received this temporary care, ensuring continuous support while their permanent providers prepare to take them on as patients.
This regional milestone relies entirely on the dedication of local primary care providers, clinics, and teams. The FLA OHT extends its sincere appreciation to the following partners for their vital role in connecting residents to care: Ontario Health @ Home - Health Care Connect, primary care partners: Amherstview Medical Care, Bath Family Clinic, Medical Tree Clinic, CDK Family Medicine, Division Street Medical Clinic, Frontenac Doctors Family Health Organization, Greater Napanee Health Home, Greenwood Medical Centre, Kingston Community Health Centres, Kingston Family Health Team, Lakelands Family Health Team, Maple Family Health Team, Midtown Kingston Health Home, Princess Street Medical Clinic, Queen’s Family Health Team, Sharbot Lake Family Health Team, Spruce Health Hub, Stone’s Mill Family Health Centre, and municipal partners: City of Kingston, Frontenac County, Loyalist Township, Town of Greater Napanee.
While this milestone marks a massive step forward, the work to attach every resident to a family doctor or nurse practitioner continues. The FLA OHT and its partners remain fully committed to expanding and improving access to primary care across the entire region.

During the first round of provincial funding, the FLA OHT and its partners exceeded their funded goal of connecting 3,500 people to care by successfully attaching more than 4,250 residents to a family doctor or nurse practitioner between April 2025 and March 2026.
