Who should attend?
Health care providers in the Ontario Health East region(communities from Pickering to the Quebec border north to Deep River) that support people navigating a life-changing diagnosis across a variety of health care settings - primary care, hospital and long-term care. Participants must be fully to attend and participate in both sessions.

Speakers:
Dr. Hsien Seow, PhD is the Canada Research Chair in Palliative Care and Health System Innovation and a professor in the Dept. of Oncology at McMaster University. He publishes health care research focused n improving the patient and family experience for those facing serious illness.
Dr. Samantha Winemaker, MD is a palliative care physician who cares for patients with serious illness and their families in the home. She is a clinical professor at McMaster University in the Dept. of Family Medicine, Division of Palliative Care. She has won numerous awards for her work.
