Now in its fourth year of operation, KHSC@Home, a partnership between two FLA OHT partners Kingston Health Sciences Centre (KHSC) and Bayshore HealthCare, has helped over 600 people – mostly but not only seniors – transition safely out of hospitals by providing up to 16 weeks of care in their homes.

In addition to helping people recover at home, the program has helped relieve hospital capacity challenges. The majority of KHSC@Home patients spend less time in the hospital and are less likely to be readmitted to a hospital or visit an emergency department after they’ve left the hospital.

The success of the program has seen it expand over the years from the Kingston area to span from Trenton to Gananoque and north of Hwy. 401. KHSC@Home is also available through FLA OHT partners Providence Care and Lennox & Addington County General Hospital.
